Use this form when you need us to release specific information to another doctor or someone you specify. Please be sure to fill out completely with the name, address and contact numbers for the person(s) you are releasing your records to. Also be very specific about the type of information you want released. We comply with HIPAA regulations and will not release information without your permission.
● Getting Medical Records Released to Dr. Young
Use this form when you need to have information released to Dr. Young from another doctor or someone else you specify. Please be sure to fill out completely with the name, address and contact numbers for the person(s) you are requesting your records from. Also be very specific about the type of information you want released. The form will instruct them to fax the information to us at 210-403-2000.
