● New Patient – Information Form
These forms can be printed out and completed prior to your Consultation visit with Dr. Young. Many of our patients like the fact that they can take their time filling them out in the comfort of their own home. It can help you form questions for the doctor, and feel better prepared. As a New Patient, there are 2 pages to complete: Patient Information; and Medical History. If you are seeing Dr. Young for a breast procedure, please complete the Breast Information Form also.
● New Patient – Medical History Form
This Medical History Form is necessary for any new patient visit you have with Dr. Young. Please fill it out completely and bring it to your Consultation visit with Dr. Young. You may attach any additional medications or information which will be helpful to us in planning a potential surgery or procedure at DrYoungForever.
● New Patient – Breast Information Form
This form will be necessary if you are seeing Dr. Young for any type of breast surgery. Please print it out and bring it to your Consultation visit with Dr. Young.

● Release of Information Form
Use this form when you need us to release specific information to another doctor or someone you specify. Please be sure to fill out completely with the name, address and contact numbers for the person(s) you are releasing your records to. Also be very specific about the type of information you want released. We comply with HIPAA regulations and will not release information without your permission.
● Getting Medical Records Released to Dr. Young
Use this form when you need to have information released to Dr. Young from another doctor or someone else you specify. Please be sure to fill out completely with the name, address and contact numbers for the person(s) you are requesting your records from. Also be very specific about the type of information you want released. The form will instruct them to fax the information to us at 210-403-2000.
