The two main types of breast implants are saline and silicone, with silicone being the more natural feeling breast. Similar breast shapes can be created with either type of implant and there is no significant difference in the complication risk for either type of implant. About Silicone Implants

Silicone implants consist of a silicone rubber shell that is filled with a predetermined measurement of silicone gel and includes a patch that covers the entry port of the implant. The silicone implants are available in a variety of shapes, profiles, volumes, textures, thicknesses, and a selection of shell lumens from which to choose. Because silicone gel has a texture that is similar to breast tissue, many surgeons feel that silicone implants have a more natural look and feel than saline implants.

In the event of a silicone implant rupture, the breast often looks and feels the same because the silicone gel may leak into surrounding areas of the breast without a visible difference. Replacing a ruptured silicone gel implant is more difficult than repairing a saline implant and silicone implants also have a higher rate of capsular contracture and higher deflation rate. The newest silicone implants are made of a cohesive gel that greatly reduces the risks associated with silicone rupture, however, they are not yet available in the United States.

After a recent comprehensive evaluation of reports that silicone breast implants were responsible for connective tissue diseases, the Association of Silicone Breast Implants and the Institute of Medicine concluded that there is ‘no definitive evidence linking breast implants to cancer, neurological diseases, neurological problems or other systemic diseases.’ Therefore, after this announcement, silicone implants have become popular once again.

Saline Implants

Saline implants are also made of a silicone rubber shell, but it is inflated to the desired size with sterile saline. Saline filled implants are available in fixed volume and adjustable volume implants, with most saline implants having a valve that is sealable by the surgeon. A fixed volume implant is filled with the entire volume of saline at the time of implantation and an adjustable volume implant is filled intra-operatively and has the potential for further postoperative adjustment. If a saline implants ruptures, it deflates and the results are almost immediately visible, whereas silicone implant ruptures are harder to detect.

Adjustable Breast Implants have been used for many years in post-cancer patients receiving implants for breast reconstruction surgery, although the same adjustment to the size of a saline implant fill or saline reduction is routine, there are decisions to make regarding what is best for the individual patient.

Breast Enlargement Techniques

There are four main techniques when it comes to a breast enlargement (augmentation) and the insertion of breast implants. The difference between them has to do with the type and placement of the incision.

Inframammary Technique

This is the most popular of the breast enlargement techniques. In inframammary breast enlargement, the implant is inserted in the fold where the breast meets the chest wall, leaving no visible scars. There is less impact on milk production in this technique because the glandular tissue is not affected.

Transaxillary Technique

This technique requires placement of an incision in the extreme upper, outer region of the breast, near the armpit to the torso. The incision will essentially be invisible even with the arm raised. The breast implants are usually placed below the muscle in this technique. The impact to milk production is usually minimal because the glandular tissue and nerves are largely undisturbed. As with the other incision techniques, placement of the implant above the muscle will result in greater impairment than placing them underneath.

Transumbillical Technique

The Transumbilical incision technique requires inserting the implant through an incision in the umbilicus (navel) and moving it into place in the breast. In this technique, no incisions are made on the breast or into the breast tissue, but the breast tissue can be disrupted and sometimes damaged as the implant is brought into position. Insertion through the umbilicus makes it difficult to position the implant accurately, requiring the use of a camera scope. It also permits placement only above the muscle. Like the Transaxillary incision technique, the Transumbilical incision technique preserves glandular function and nerve response so that the impact to milk production is usually slight. As with the other incision techniques, placement of the implant above the muscle will result in greater impairment than placement below.

Periareolar Technique

The Periareolar Incision technique requires an incision around the areola. Surgeons often use this technique to hide scarring. Placement of the implant in this location may result in a considerable duct, glandular, and nerve damage, carrying significant risk to milk production. Ducts and glands are likely to be severed because the incision penetrates deeply through the breast tissue. If the implant is placed above the muscle, it may further impede milk production functionality by placing pressure upon the glandular tissue. This is the least popular augmentation technique and should be carefully considered and discussed with Dr. Young.
